A Tesla electric car unexpectedly broke down in the crowded Bay Tunnel in San Francisco, resulting in an 8-car collision last month.
Nine people were injured in the accident, according to a report from the California Highway Patrol.
Surveillance footage from the tunnel went viral on Tuesday, showing the Tesla car changing lanes and slowing to a stop in the middle of a congested road.
As a result, several drivers had to stop quickly behind her, but others behind them did not stop and a serious collision ensued.
In their report, the California Highway Patrol said they could not confirm whether "fully autonomous driving" was active at the time of the accident.
